Our case managers provide personalized support to help you navigate challenges and move steadily along your road to self-sufficiency. We act as a bridge—filling gaps as they arise and helping you maintain and grow your employment, manage resources, and plan for a stronger future.


Types of Help Our Case Managers Provide:

  • Personalized Needs Assessment: Understand your unique challenges and goals
  • Goal Setting & Planning: Create step-by-step plans for progress toward self-sufficiency
  • Resource Referrals: Connect you with community services like food assistance, healthcare, and childcare, workforce training, and more
  • Financial Support Guidance: Offer guidance on budgeting and managing short-term financial challenges
  • Employment Support: Provide limited employment-related support such as transportation assistance (including minor car repairs, gas cards, bus passes, Uber, Lyft, and other rideshare services) to avoid job loss
  • Laundry Assistance: Provide limited support for laundry access when lack of clean clothing may affect employment or daily functioning
  • Proactive Support: Monitor and anticipate potential barriers before they impact your progress
  • Advocacy: Act as your advocate to help you access the services you need
  • Consistent Follow-Up: Regular check-ins to adjust plans and provide ongoing encouragement
  • Youth Support: Specialized case management for adolescents (ages 16–19) working toward early self-sufficiency
